The column "type status" is being used to add information about the certification status of the centre, but in the edit form it is labelled as "Comments about centre's type". I suggest to change this label to something closer to its actual use, e.g. "certification status".
To avoid inconsistencies, it would also be nice to change the field into a semi-controlled vocabulary like "assessment date" where you can choose from a pre-defined list, but can still add new values to this list through the UI.
