There is an error in pm2-zabbix.template.xml.
In all <item_prototype> after <name> replace $1 with {#PROCESS_NAME}

  <item_prototypes>
      <item_prototype>
          <name>$1 CPU Usage</name>

or change it her:
image

And even better change it to something like: <name>PM2 {#PROCESS_NAME} CPU Usage</name>
