Duplicate video facilitator guides

We currently have two slightly different versions of the video facilitator guides in https://github.com/open-life-science/policies-procedures-and-docs/blob/main/cohort-procedures-and-templates/Cohort-call-facilitator-Guide.md and PR #31. We need to decide which version we keep.
I would prefer to keep the video and call facilitator guides separate.
