Update to support latest QGIS and PostgreSQL

This release updates the plugin to support recent QGIS (2.18) and PostgreSQL (9.6) versions specifically for a new deployment in Niger State, Nigeria by Spatialmatrix in May 2017. It is a temporary release while v3.0 is completed.
